- Oct 8, 2026, 16:04 UTCmonitoringWe want to make you aware of an issue under investigation involving sensorimotor exam information in ICP. Reported behavior includes measurements in the Sensorimotor Grid not being retained as entered or reverting to previously entered values. Saving delays have also been reported. As a result, sensorimotor information displayed in the chart may not accurately reflect the intended entries. While we investigate, please verify sensorimotor measurements against the original exam findings or another reliable clinical source before relying on them for clinical decisions. If information is missing, unexpected, or inconsistent with the exam findings, please do not rely on those values until they have been verified, and contact Nextech Support for assistance. We recognize the importance of accurate clinical documentation. Our team is actively investigating the reported behavior to determine its cause, scope, and appropriate resolution. We will share additional guidance and updates as more information becomes available. Please contact Nextech Support if you observe this behavior or have questions. Your reports help us investigate and support affected practices. Thank you for your attention and partnership.

- Oct 15, 2026, 18:52 UTCmonitoringThe issue involving incorrect measurement values in the Sensorimotor Grid has been corrected by the update deployed on October 8, 2026. This correction does not validate previously documented, potentially affected measurements. Practices identified as potentially affected have received direct communication and encounter-specific review guidance. Please continue to follow that guidance and verify potentially affected measurements against the original exam findings or another reliable clinical source before relying on them for clinical decisions or copying them forward. If you have questions or observe unexpected sensorimotor values, please contact Nextech Support. Thank you for your patience and partnership.
